Tumor Markers: Do Not Panic Yet

I know. You got a tumor marker result back and your brain immediately went to the worst-case scenario. Before you spiral, let me tell you something important: elevated tumor markers do not automatically mean cancer. Seriously. There are dozens of non-cancerous reasons these numbers go up.

Tumor markers are substances produced by cancer cells or by your body in response to cancer. But they are also produced during infections, inflammation, pregnancy, smoking, and plenty of other normal situations. That is why doctors rarely use them alone to diagnose cancer. They are one piece of a much bigger puzzle.

Common Tumor Markers Tested

  • PSA (Prostate-Specific Antigen) - prostate cancer screening, but also elevated with benign prostate enlargement and infections
  • CA-125 - associated with ovarian cancer, but also rises with endometriosis, fibroids, and even menstruation
  • CEA (Carcinoembryonic Antigen) - linked to colorectal cancer, but smokers often have elevated levels
  • AFP (Alpha-Fetoprotein) - liver cancer marker, also elevated during pregnancy and with liver disease
  • CA 19-9 - associated with pancreatic cancer, also rises with bile duct blockages
  • CA 15-3 - used to monitor breast cancer treatment
  • HCG (Human Chorionic Gonadotropin) - pregnancy hormone, but also a marker for certain testicular and ovarian cancers
  • LDH (Lactate Dehydrogenase) - very non-specific, elevated in many cancers and non-cancerous conditions

How Tumor Markers Are Actually Used

Most tumor markers are better for monitoring than screening. If you have been diagnosed with cancer and are undergoing treatment, falling tumor marker levels are a good sign. Rising levels might mean the treatment needs adjusting. For initial screening, only PSA is widely used, and even that is debated.

The exception is if you have a strong family history or known genetic risk factors. In those cases, your doctor might track certain markers over time to catch changes early.

When to Get Tumor Markers Tested

Most people do not need routine tumor marker testing. These tests are typically ordered when cancer is suspected based on symptoms or imaging, when monitoring treatment response, or when checking for recurrence after treatment. If your doctor ordered one as part of a routine health screening (common in some Asian countries), an elevated result is not a diagnosis. It is a flag that needs follow-up.
